29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about September 16, 2020, at a residential construction site in Tomball, TX, employees were exposed to a struck-by hazard while operating nail guns without wearing eye protection.

29 CFR 1926.451(b)(3): The front edge of all platforms was not 14 inches (36 cm) or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling.   On or about September 16, 2020, at the construction site, an employee was exposed to a fall hazard while working on a pump jack scaffold whose front edge was more than 14 inches (36 cm) from the face of the work.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:    On or about September 16, 2020, at the construction site, an employee was exposed to a fall hazard while working from a pump jack scaffold that did not have an adequate foundation on base plates and mud sills that would prevent the scaffold from settling into the ground.

On or about September 16, 2020, at the construction site, an employee was exposed to a fall hazard while working from a pump jack scaffold that was not secured by rigid bracing.

On or about September 16, 2020 on the construction site, an employee was exposed to a fall hazard of more than 6 feet while building a cornice from a scaffold without any type of a fall protection system.
