29 CFR 1910.133(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: a) Denver Reel and Pallet Company, located at 4600 Monaco Street, Denver CO 80216: On or before September 17, 2020, the employer did not ensure that employees who were using pneumatic nails guns to assemble wooden pallets and shipping crates were wearing eye or face protection. This condition exposed employees to the hazard of projectiles to the eyes. b) Denver Reel and Pallet Company, located at 4600 Monaco Street, Denver CO 80216: On or before September 17, 2020, the employer did not ensure that employees who were transferring 10% bleach from 55 gallon drums to secondary containers, used eye or face protection. This condition exposed exposed employees to the hazard of caustic liquids to the eyes and face.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operators and other employees from hazards created by ingoing nip points, rotating parts, flying chips and sparks: a) Denver Reel and Pallet Company, located at 4600 Monaco Street, Denver CO 80216: On or before September 17, 2020, the employer did not ensure that the saw blade in the machine used to cut wood, framing stakes was guarded, in that the opening at the discard chute at the back of the machine was large enough to allow for operators to place their hand inside while the saw blade was turning. The saw blade continues to spin during a spool down period even after the machine is turned off and/or unplugged. Operators have to access this area to remove the cut wood pieces. This condition exposed employees to laceration and amputation hazards. b) Denver Reel and Pallet Company, located at 4600 Monaco Street, Denver CO 80216: On or before September 17, 2020, the employer did not ensure that the saw blade in the machine used to cut wood stakes was guarded, in that the opening at the front side of the machine was large enough to allow for operators to inadvertently place their hand inside while cutting stakes. This condition exposed employees to laceration and amputation.

The employer had a flexible cord running from an electrical box, that was suspended from the ceiling in the work area. The flexible cord was being used as fixed wiring of the structure. This condition exposed employees to shock and electrical hazards.
