29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  At a worksite located at 9500 Grand Ave Kansas City, MO: Employees were exposed to struck by hazards. This was demonstrated when workers were observed working with a pneumatic nail guns without the use of personal eye protection. The employer failed to ensure that employees were provided and used the necessary personal protective equipment to perform their work safely.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   At a worksite located at 9500 Grand Ave, Kansas City, MO: Employees were exposed to a fall hazards. This was demonstrated when workers were observed working at heights of approximately 7 and 13 feet without the use of fall protection, to include, guardrail systems, safety net systems, or personal fall arrest systems. The employer failed to ensure that employees were provided  and used personal protective equipment needed to perform their work safely.
