29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about 09/16/2020, at the north side of a residential home roof, a crew of employees were conducting roofing work without a means of fall protection: a) at a height of approximately 16 feet on the roof. b) at a height of approximately 12 feet on a 2 X 6 planks on the roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about 09/16/2020, on the northside of a residential home roof, employees were conducting roofing work, and were using an extension ladder to access/egress the roof, without the ladder being 3 feet above the eave/landing.
