29 CFR OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to amputation hazard while working without any lighting: a. On or about September 10, 2020, at 2300 W Airport Blvd, Sanford FL, employees were exposed to an amputation hazard while attaching a trailer on the hitch of a truck when working without lighting and feeling the ball with their hand to make sure it is lined to engage properly.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a. On or about 6:45 AM on September 10, 2020, at 2300 W Airport Blvd, Sanford, FL, 32771, an employee suffered a work-related finger amputation. The employer was aware of the injury on September 10, 2020. The employer notified OSHA on September 15, 2020 at 8:13 AM of the amputation.
