1926.1424(a)(3)(i) Before an employee went to a location in the hazard area that was out of view of the operator, the employee(or someone instructed by the employee) did not ensure that the operator was informed that he/she was going to that location: Location: Jobsite. Instance 1. On or about September 22, 2020 employees were exposed to overhead struck-by hazards while working where bundles or Isopoll roof insulation were not rigged to prevent unintentional displacement. Instance2. September 22, 2020 Accident. An employee was seriously injured when a bundle of Iso insulation became displaced during a hoisting operation and struck the employee on the upper back.
