DnR Construction, LLC, on or about September 23, 2020 at the address of 6855 Spring Valley Dr. Holland, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as preventing falls from heights on the worksite.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) DnR Construction, LLC, on or about September 23, 2020 at the address of 6855 Spring Valley Dr. Holland, OH: The employer did not ensure employees were protected from falls while performing roofing activities for a commercial type structure. Employees were exposed to approximately 12 ft. falls to the ground below.
