29 CFR  1926.20(b)(2):The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:  a. At 11416 Alachua Creek Lane, Riverview, FL, employees were exposed to hazards relating to roof sheathing at residential construction sites, in that, safety inspections were not conducted to assure proper use of fall protection equipment, on or about September 16, 2020.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. At site located on 11427 Alachua Creek Lane in Riverview, Florida, on or about September 16, 2020, employees  engaged in residential roof sheathing operations while standing on roof trusses were exposed to a 12 foot fall hazard without using any type of fall protection.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:  a. At a construction site located on 11427 Alachua Creek Lane in Riverview, Florida, on or about September 22, 2020, employees are not provided training and education relating to fall protection, hazard recognition, procedures to be followed to minimize these hazards and the proper use and limitation of fall protection equipment and fall protection systems. An employee performing residential roof sheathing on the backyard Lanai at a height of 12 feet, fell  to the ground and sustained serious injuries.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a. On or about 12:00 PM on September 16, 2020, at 11416  Alachua Creek Lane, Riverview, FL, 33579, an employee suffered a work related injury and was hospitalized at 12:00 pm of September 16, 2020. The employer was aware of the in-patient hospitalization in the afternoon of September 16, 2020. The employer notified OSHA on September 18, 2020 at 4:01 PM of the in-patient hospitalization.
