29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting: There was no training on whether employees were to use portable fire extinguishers or only to evacuate. Training was not provided on the use of portable fire extinguishers. (Note: the emergency evacuation route plan, did not include a meeting place for employees once they exited the building.) 29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: At the warehouse cutting tables, equipment, including cooling fans and the cutting table were powered with multiple extension cords and power taps plugged into each other. (Note: power taps were required to be connected directly to building wiring outlets or drop down outlets. The surge manual states: "Connect the UPS power cable directly to a wall outlet." The manual also requires that not to operate with dust buildup, "excessive dust..." At the time of the inspection lint/dust was laying on the power taps and cord connections.) Abatement documentation is require for this violation.
