29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. a) Aspen Group Co Inc., at Tava Waters, 9099 E Mississippi Ave, Denver, CO 80247 : On or before September 29, 2020, the controlling employer did not ensure that subcontractor employees were protected from eye hazards, in that employees using pneumatic nail guns on the roof of multiple structures to secure roof shingles, were not using eye protection. This condition exposed subcontractor employees to flying debris and eye injuries.

29 CFR 1926.502(j)(7)(i): Materials and equipment were being stored within 6 feet (1.8 m) of a roof edge without guardrails erected at the edge. a) Aspen Group Co Inc., at Tava Waters, 9099 E Mississippi Ave, Denver, CO 80247 : On or before September 29, 2020, the controlling employer did not ensure that subcontractor employees were protected from falling objects, in that stacks of shingles were placed within 6 feet of the roofs edge without a guardrail system in place. This condition exposed employees to falling objects from elevated heights.
