29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a)  Main Roof -  Employees were observed conducting roofing operations on a low-slope residential roof and were not protected from an approximate 25-foot fall hazard to the ground below, on or about September 29, 2020.  b)  Porch Roof - Employees were observed conducting roofing operations on a steep porch roof and were not protected from fall hazards no less than 8 feet - 11 inches to the ground below, on or about September 29, 2020.  Abatement certification and documentation required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)  Jobsite - Employees were not trained in appropriate fall protection methods to avoid hazards associated with elevated work areas, on or about September 29, 2020.  Abatement certification and documentation required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Main Roof - Employees used a 28-foot Werner extension ladder to access the main roof and the side rails extended approximately 1-1/2 feet (one and a half feet) above the landing surface, on or about September 29, 2020.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
