29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  Worksite: The employer did not ensure that the employees who were working on the flat commercial building roof were protected from fall hazards above 6 feet by the use of guardrails, safety nets, personal fall arrest systems or a warning line system.  AEC Zecena, LLC was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA Inspection Number 1356875, Citation Number 1, Item Number 2a and was affirmed as a final order on 12/27/2018, with respect to a workplace located at 24 Norris Street, Waterbury, CT.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  Worksite: The employer did not ensure that the employees who were exposed to fall hazards while working on the roof at a height above 6 feet were trained in fall hazards and the procedures to be followed to minimize such hazards.  AEC Zecena, LLC was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA Inspection Number 1356875, Citation Number 1, Item Number 2b and was affirmed as a final order on 12/27/2018, with respect to a workplace located at 24 Norris Street, Waterbury, CT.
