29 CFR  1926.451(b)(1): Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  On 9/24/2020, employees of Wolf Creek Plastering & Stone, LLC were performing masonry work on a building located at 339 J Mark Court, OFallon, MO.  The employees were working from a scaffold platform that was not fully planked or decked.

29 CFR  1926.451(e)(1):  When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used. Crossbraces were used as a means of access:  On 9/24/2020, employees of Wolf Creek Plastering & Stone, LLC were performing masonry work on a building located at 339 J Mark Court, OFallon, MO.  Employees did not have sufficient scaffold access to the upper working levels of the scaffold system.  Employees were climbing on the crossbracing to access the lower and upper levels.

29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  On 9/24/2020, employees of Wolf Creek Plastering & Stone, LLC were performing masonry work on a building located at 339 J Mark Court, OFallon, MO. Employees were not trained by a qualified person in hazard recognition for the tube welded scaffold.  The employees were exposed to fall hazards.

1926.451(g)(1):  Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level:  On 9/24/2020, employees of Wolf Creek Plastering & Stone, LLC were performing masonry work on a building located at 339 J Mark Court, OFallon, MO.  The employees were working from a tubular welded scaffold system approximately 14 feet above the ground.  Employees were not protected from falling to the lower level.  Wolf Creek Plastering & Stone LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 1257380, citation number 1, item number 1 and was affirmed as a final order on 11/21/2017, with respect to a workplace located at 9630 Rosehill Road, Lenexa, Kansas.
