29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) Jobsite: On or about September 30, 2020, an employee was working (installing an 8 inch diameter plastic water line pipe) in an excavation that was 5 feet 6 inches (including 4 inches of sand in the bottom of the bottom of the trench) in depth and there was no ladder, ramp or other safe means to exit the excavation. The excavation was about 40 feet in length.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): (a) Jobsite: On or about September 30, 2020, an employee was not protected from a sidewall cave-in while he was working in a 5 foot 6 inch deep trench. The company was installing 22 foot long sections of 8 inch diameter plastic water piping.
