On or about September 28, 2020, at Sanibel Causeway, Fort Myers, FL, an employee was exposed to a struck-by hazard while placing a hand held wrench on a rotating rod of a horizontal directional boring machine during the pre-reaming process.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a. On or about September 28, 2020, at Sanibel Causeway, Fort Myers, FL 33957, employees were exposed to horizontal directional boring operations without being provided proper training and education relating to the hazards of the boring equipment the rotating drill string and the hazard of using hand held tools on rotating equipment.
