29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about September 28, 2020, an employee was exposed to fall hazards of approximately 20 feet while performing roofing activities on a two story residential home and the employer did not ensure a suitable fall protection system was used.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:       On or about September 28, 2020, an employee was exposed to fall hazards of approximately 20 feet while accessing and exiting the roof via an extension ladder which did not extend 3 feet above the roof being accessed.
