29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about October 1, 2020, during the replacement of a fire hydrant at a worksite in the road across from 2525 Royalton Road, Broadview Heights, Ohio, the employer did not ensure two employees working inside a trench, at a depth of approximately 7 to 8 feet, were protected from cave-ins by an adequately designed protection system. The soil would have been classified as; Cohesive Soil, Type B .
