On or about October 1, 2020, during the replacement of a fire hydrant at a worksite in the road across from 2525 Royalton Road, Broadview Heights, Ohio, the employer did not ensure two employees working inside a trench, at a depth of approximately 7 to 8 feet, were protected from cave-ins by an adequately designed protection system.  The soil would have been classified as; Cohesive Soil, Type B.
