29 CFR  1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) MKS Instruments, Inc. at 11380 Reed Way, Broomfield, CO: On and before October 1, 2020, the employer had developed but had not documented Energy Control Procedures or ensured that the Energy Control Procedures were utilized for a Horn Machine Tools Horizontal Bending Machine. A machine operator was positioning a pipe with his left hand in a point of operation of the Horn Machine Tools Horizontal Bending Machine, when he unexpectedly energized the mechanical systems of the machine at an adjacent control panel with his right hand and his left hand was crushed between the pipe and machine.  The machine was energized with electrical and pneumatic systems. This condition exposed the employee to point of operation and amputations hazards and resulted in the surgical amputation of three fingers on his left hand.   Abatement Note:  Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.  The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: 1)	A specific statement of the intended use of the procedures; 2)	Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; 3)	Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and 4)	Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.

29 CFR  1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) MKS Instruments, Inc. at 11380 Reed Way, Broomfield, CO: On and before October 1, 2020, the employer did not ensure that several metal electrical outlet boxes were used in accordance with listed instructions, in that the outlet boxes in use on the flexible cords included pre-punched knockouts that could be dislodged by frequent handling or being struck-by objects in Model Shop 171. This condition exposed employees to electrical hazards while connecting and disconnecting electric-powered equipment at the outlet box.  b) MKS Instruments, Inc. at 11380 Reed Way, Broomfield, CO: On and before October 1, 2020, the employer did not ensure that a metal electrical outlet box was used in accordance with listed instructions, in that the outlet box in use that was fastened to the Horn Machine Tools Horizontal Bending Machine included pre-punched knockouts that could be dislodged by frequent handling or being struck-by objects. This condition exposed employees to electrical hazards while connecting and disconnecting electric-powered equipment at the outlet box.
