29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low sloped roofs, with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guard rail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. On or about October 1, 2020, and at times prior thereto, an employee performing roofing activities and removing vent boots from the rooftop of a commercial building was not provided fall protection. The employee was exposed to an unprotected fall hazard of approximately 12-14 feet to the concrete below.

29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways:( On or about October 1, 2020, and at times prior thereto, an employee laid an extension ladder on a medal roof supported by the scissor lift to work on the roof area and to remove vent boots from the rooftop of a commercial building. The employee was not provided training on proper use of ladders and exposed to an unprotected fall hazard of approximately 12-14 feet to the concrete below.
