The machine was not deenergized while employee(s) conducted die and raw material changes, exposing the employee(s) to ingoing nip point, crushing hazard and amputation injury.

1910.212(a)(1): Machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts and crushing hazards. (a) Jesgel 850 ton injection molding machine (# EW1858): On or about Sept 24, 2020, the employer did not insure guarding was properly installed and implemented on the machine feed hopper exposing employee(s) working on or around the energized, exposed and rotating feed screw and trough housing located directly below the hopper. This exposed employees to ingoing nip points, rotating parts and amputation injury. In the alternative... 1910.147(a)(2)(ii)(A): Lockout procedures were not implemented when servicing and/or maintenance was taking place as part of machine setup operations or operations when an employee was required to bypass or remove a guard, or other safety device. (a) Jesgel 850 ton injection molding machine (#EW1858): On or about Sept 24, 2020,the employer did not insure machine cell was deenergized and locked out when employee removed hopper shroud to perform a material purge / color change, as part of servicing of production process equipment. This exposed employee(s) working on or around the exposed feeder screw for the injection molding machine to ingoing nip point, rotating parts, and amputation injury.
