29 CFR  OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that workers were exposed to struck-by hazards.  Stripping Barn, On or about October 1, 2020,  workers were exposed to a struck by hazard, in that, pallets were stacked where they were stacked at a height resulting in them leaning, not secured, and unprotected in passageways where employees travel and operate a powered industrial truck exposing them to being struck-by falling pallets.  Among other methods, one feasible and acceptable means of abatement would be to stack and secure pallets at a height to ensure they are stable and secure .

29 CFR  OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that sprocket and chains on the stripping chains  were not protected from pinch points with machine guarding to prevent employee injury.  a) Stripping Barn - Stripping Chain 1: On or about October 1, 2020,  workers removing tobacco leaves from tobacco stalks were exposed to being caught-in the unguarded chains and sprockets on stripping chains.  b) Stripping Barn - Stripping Chain 2: On or about October 1, 2020,  workers removing tobacco leaves from tobacco stalks were exposed to being caught-in the unguarded chains and sprockets on stripping chains.  c) Stripping Barn - Stripping Chain 3: On or about October 1, 2020,  workers removing tobacco leaves from tobacco stalks were exposed to being caught-in the unguarded chains and sprockets on stripping chains.  d) Stripping Barn - Stripping Chain 4: On or about October 1, 2020,  workers removing tobacco leaves from tobacco stalks were exposed to being caught-in the unguarded chains and sprockets on stripping chains.  e) Stripping Barn - Stripping Chain 5: On or about October 1, 2020,  workers removing tobacco leaves from tobacco stalks were exposed to being caught-in the unguarded chains and sprockets on stripping chains.  f) Stripping Barn - Stripping Chain 6: On or about October 1, 2020,  workers removing tobacco leaves from tobacco stalks were exposed to being caught-in the unguarded chains and sprockets on stripping chains.  Among other methods, one feasible and acceptable means of abatement would be to guard the chain and sprockets on the stripping chains.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that workers were exposed to electrocution and electrical shock.  a) Stripping barn - northwest corner: On or about October 1, 2020, the dead front of the electrical panel was removed exposing workers to an electrical shock hazard.  b) Stripping barn - northwest corner: On or about October 1, 2020, the wiring on the motor assembly for the Stripper Chain was pulled loose exposing employees to electrical shock while removing leaves from tobacco stalks.  c) Stripping barn - northwest corner: On or about October 1, 2020, outlet covers were missing on outlets along west wall exposing workers to an electrical shock when plugging in scales to weigh the tobacco.     d) Stripping barn - northwest corner: On or about October 1, 2020  an outlet cover was missing on an electrical outlet along northwest wall exposing workers to electrical shock when plugging in stripping chains.  e) Stripping barn - north wall: On or about October 1, 2020 an outlet cover missing on an electrical outlet along west and north wall exposing workers to electrical shock when plugging in stripping chains.    Among other methods, one feasible and acceptable means of abatement would be to place a dead front cover on the electrical panel, secure the wiring in the junction box, and install covers on the electrical outlets.
