29 CFR  1910.146(c)(5)(ii)(E)(2): Employees entered into permit spaces but the forced air ventilation was not so directed as to ventilate the immediate areas where an employee was or would be present and continued until the last employee left the space:  On or about 09/30/2020, at 3400 SE 32 Avenue, Fort Lauderdale, Florida, employer did not provide continuous forced air ventilation while an employee was preforming cleaning to the inside of a fuel tank, thus exposing employee to an asphyxiation hazard.

29 CFR  1910.146(c)(5)(ii)(F): The atmosphere within the space was not periodically tested as necessary to ensure that the continuous forced air ventilation was preventing the accumulation of a hazardous atmosphere.  On or about 09/30/2020, at 3400 SE 9 Avenue, the employer did not ensure the atmosphere in the confined space inside a fuel tanker was monitored for continuous forced air ventilation to prevent a hazardous atmosphere, thus exposing  employees to an asphyxiation hazard.
