(a)  Valley View Elementary:  On or about September 25, 2020, and at times prior thereto, the controlling contractor, Beniton Construction, Inc. did not ensure its sub-contractor protected its employees while performing structural steel activities on the metal deck.  Four open penetrations were not covered and/or permanently filled, exposing the sub-contracted employees to trip and/or fall hazards to a concrete floor below.
