29 CFR  1910.269(d)(2)(i): The employer shall establish a program consisting of energy control procedures, employee training, and periodic inspections to ensure that, before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, start up, or release of stored energy could occur and cause injury, the machine or equipment is isolated from the energy source and rendered inoperative.   a) On 9/26/20, employees were exposed to electrical hazards of 1500 volts dc while cutting out damaged connectors  for solar panels located at row IS18.  Employer did not ensure that the general program consisted of energy control procedures, employee training and periodic inspections.

29 CFR  1910.332(b)(3):Additional requirements for qualified persons. Qualified persons (i.e. those permitted to work on or near exposed energized parts) shall, at a minimum, be trained in and familiar with the following:  1910.332(b)(3)(i) The skills and techniques necessary to distinguish exposed live parts from other parts of electric equipment.  1910.332(b)(3)(ii) The skills and techniques necessary to determine the nominal voltage of exposed live parts, and  1910.332(b)(3)(iii) The clearance distances specified in 1910.333(c) and the corresponding voltages to which the qualified person will be exposed.    Note 1: For the purposes of 1910.331 through 1910.335, a person must have the training required by paragraph (b)(3) of this section in order to be considered a qualified person.    Note 2: Qualified persons whose work on energized equipment involves either direct contact or contact by means of tools or materials must also have the training needed to meet 1910.333(C)(2).   a) On or about 9/26/2020 the employer did not ensure that unqualified persons acting as  lead electrician, installers and laborers were properly trained to have the knowledge to perform the task such as but not limited to installing solar panels and connecting electrical harnesses did not receive training prior to performing the tasks on 1910.332(b)(3)(i), 1910.332(b)(3)(ii) & 1910.332(b)(3)(iii) of the standard.

29 CFR  1910.333(c)(2):"Work on energized equipment." Only qualified persons may work on electric circuit parts or equipment that have not been deenergized under the procedures of paragraph (b) of this section. Such persons shall be capable of working safely on energized circuits and shall be familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools.  a) On or about 09/26/20 the employer did not ensure employees working on energized equipment such as but not limited to the installation of solar panel connectors and disconnecting damaged connectors, had the additional PPE or knowledge and skills of a qualified person to perform such task safely.

29 CFR  1910.335(a)(1)(i):Employees working in areas where there are potential electrical hazards shall be provided with, and shall use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed.  Note: Personal protective equipment requirements are contained in subpart I of this part.  a) On or about 9/26/20 the employer did not provide electrical rated protective equipment such as but not limited to; protective shields, protective barriers or insulating materials for employees installing solar panel connectors.  Employee were exposed to electrical shock of 1500 volts dc.
