29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a. Eldorado Training Company II, Inc. at 2325 W Midway Blvd, Broomfield, CO 80020: On and before October 5, 2020, the employer did not establish a program of consisting of energy control procedures and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated from the energy source, and rendered inoperative. The employer assigned a worker to lubricate a roller conveyor but did not ensure it was isolated and inoperative, and the worker sustained a finger amputation when the conveyor unexpectedly moved during the task. Abatement Note: Abatement certification and documentation are required for this item (See enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section: a. Eldorado Training Company II, Inc. at 2325 W Midway Blvd, Broomfield, CO 80020: On and before October 5, 2020 machine specific, step-by-step procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in maintenance of the TGW Systems XenoROL � conveyors XR40 and XR48 IOM, model #M4467793. On September 23, 2020, an employee sustained a finger amputation while performing maintenance without first rendering the machine inoperable. This condition exposed employees to amputation hazards. Abatement Note: Abatement certification and documentation are required for this item (See enclosed "Certification of Corrective Action Worksheet").

Employees maintained and operated the TGW Systems XenoROL � conveyors XR40 and XR48 IOM, model #M4467793 without the line-shaft guards not in use which prevent entanglement in rotating parts. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a. El Dorado Trading Company II, Inc. at 2325 W Midway Blvd, Broomfield, CO 80020: On and before October 5, 2020, the employer failed to report within 24-hours a work-related incident resulting in an amputation. The employer did not report a finger amputation that occurred on September 23, 2020, while the employee was conducting maintenance to a conveyor. Abatement Note: Abatement certification is required for this item (See enclosed "Certification of Corrective Action Worksheet").
