The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a. Eldorado Training Company II, Inc. at 2325 W Midway Blvd, Broomfield, CO 80020: On and before October 5, 2020, the employer did not establish a program of consisting of  energy control procedures and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated from the energy source, and rendered inoperative. The employer assigned a worker to lubricate a roller conveyor but did not ensure it was isolated and inoperative, and the worker sustained a finger amputation when the conveyor unexpectedly moved during the task.
