29 CFR 1926.20(b)(2):  Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:  On 10/2/2020, Employees of Esperanza Contracting LLC were performing roofing work on a residential building at 908 Gainesway Court, Florissant, Missouri.  A competent person did not conduct frequent and regular inspections of the jobsite.  Employees on the site were exposed to hazards including but not limited to fall hazards and struck-by hazards.

29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On 10/2/2020, employees of Esperanza Contracting LLC were performing roofing work at 908 Gainesway Court, Florissant, Missouri. Pneumatic nail guns were being used by employees without the use of eye protection, exposing them to a struck-by hazard.

Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems:  On 10/2/2020, employees of Esperanza Contracting LLC were performing roofing work at 908 Gainesway Court, Florissant, Missouri.  Workers were removing and installing new shingles approximately 18 feet above a lower level.  Workers did not implement a guardrail systems, a safety net system, or a personal fall arrest system to eliminate fall hazards.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee who might be exposed to fall hazards. The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards.  On 10/2/2020, employees of Esperanza Contracting LLC were performing roofing work at 908 Gainesway Court, Florissant, Missouri.  The roofing workers were not able to recognize fall hazards and the procedures necessary to reduce or eliminate these hazards.
