29 CFR 1926.300(b)(1): Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use: (a) On or about October 6, 2020 and at times prior to that date, for the employees exposed to amputation and/or struck-by hazards where a Makita angle grinder, model no. 9557NB, serial no. 2302918 equipped with a cut off wheel was operated with the guard removed at FY19 Combat Rescue Helicopter Simulator Facility, Nellis AFB, Nevada.
