(a) Scott's Roofing, at 1131 S. Sherman Street, Denver, CO 80210: On or before October 7, 2020, the controlling employer did not ensure that subcontractor employees who were using pneumatic nails guns to affix roof shingles, were wearing eye or face protection. This condition exposed employees to the hazard of projectiles to the eyes.

(a) Scott's Roofing, at 1131 S. Sherman Street, Denver, CO 80210: On or before October 7, 2020, the controlling employer did not ensure that subcontractor employees were protected from injury, in that employees actively engaged in roofing activities on a residential townhome and were not utilizing a fall protection system. This condition exposed employees to a fall hazard of 20-40 feet.

29 CFR 1926.1053(b)(1): Portable ladders was used for access to an upper level landing surface and the ladder side rails did not extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. (a) Scott's Roofing, at 1131 S. Sherman Street, Denver, CO 80210: On or before October 7, 2020, the controlling employer did not ensure that the ladder subcontractor employees were using to access the upper landing surfaces extended 3 feet above the upper landing areas nor was it secured at its top. 6 subcontractor employees engaged in re-roofing activities on a townhome, did not have their extension ladder adjusted correctly so that the side rails extended at least 3 feet above the work surface or was secured at its top. This condition exposed the employees to a fall hazard of 40 feet.
