29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary: (a) East Side of New Building - Employees worked from mast climbing scaffolds, erected end to end along the building's east side, to install the brick facing on the building. A gap of about 8 inches existed between the main platform level on the Premier mast climber and the adjacent Hydro Mobile mast climber, exposing employees to the hazards of tripping in or stepping into or through this gap, on or about October 6, 2020. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.451(b)(7): On scaffolds where platforms are overlapped to create a long platform, the overlap was not at least 12 inches (30 cm) and the platforms were not nailed together or otherwise restrained to prevent movement: (a) East Side of New Building - Employees worked from mast climbing scaffolds, erected end to end along the building's east side, to install the brick facing on the building. The western most plank, not nailed or otherwise restrained, used to bridge the outrigger platform level between the Premier mast climber and the adjacent Hydro Mobile mast climber, extended an inch or two over the outrigger support on the Hydro Mobile mast climber, making this plank susceptible to sliding off the Hydro Mobile's outrigger support and exposing employees to the hazard of falling 13 feet to the ground below, on or about October 6, 2020. No abatement certification required.

On the Premier mast climber, the front mud sill on the northern most tower/mast was uneven, sloping downward from south to north, on or about October 6, 2020. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
