29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  Jobsite:  On or about October 7, 2020, two employees were working (installing ice guard) on a residential roof and inadequately protected from falling 14 feet to the ground.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill:  (a)  Jobsite:  On or about October 7, 2020, two employees need to be retrained on when to use fall protection when working from a roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)  Jobsite:  On or about October 7, 2020, two employees were using an aluminum extension ladder to access the roof, and the ladder only extended 1 1/2 feet above the edge of the roof.  The fall hazard to the employees was about 14 feet to the ground.

29 CFR 1926.1060(b): The employer did not provide retraining as necessary for each employee using ladders and stairways so that the employee maintained the understanding and knowledge acquired through prior training:  (a)  Jobsite:  On or about October 7, 2020, the employer needs to retrain its two employees on how to properly use ladders.
