29 CFR 1926.502(f)(1)(i):When no mechanical equipment was being used the warning line was not erected at least 6 feet (1.8m) from the roof edge. a) On or about October 7, 2020, at the previously addressed job site, employees were performing a roof tear off on a flat roof with a warning line system that was erected less than 6 feet from the roof edge. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.502(f)(2)(ii):The rope, wire, or chain was not rigged or supported in such a way that its lowest point (including sag) was not less than 34 inches (0.9 m) from the walking/working surface and its highest point no more than 39 inches (1.0 m) from the walking/working surface. a) On or about October 7, 2020, at the previously addressed job site, employees were performing a roof tear off with a warning line system erected. The warning line was less than 34 inches from the walking/working surface because it was touching the walking/working surface (roof) and/or sagging over the edge of the walking/working surface (roof) at different locations. The hazard was corrected during the inspection.
