29 CFR  1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition.  Sources of ignition may include open flames, lightning, smoking, hot sirfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat:  a) Welding Area: On or about October 7, 2020, repair work such as grinding and welding were performed on used boat fuel tanks. Precautions were not taken to prevent the ignition of residual flammable vapors of the remaining fuel in the tank.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) K&F Drill Press:  On or about October 1, 2020 and times prior, employees used a K&F to drill holes in metal parts.  The rotating bit was not provided with a guard to prevent employee exposure to the flying chips and rotating parts.  b) Baileigh Lathe: On or about October 1, 2020 and times prior, employees used a Baileigh lathe to do light machine work.  The rotating chuck was not provided with a guard to prevent employee exposure to the rotating parts.  c) Gear Press: On or about October 1, 2020 and times prior, employees used a gear press to bend metal parts. The machine was not guarded to protect employees from the pinch point/crushing hazards.

29 CFR 1910.1200(e)(1):   The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    (a) Custom Welding & Fabricating : On or about October 7, 2020 and times prior, employees used chemicals such as cutting oil, oxygen and acetylene during metal fabricating operations.  The employer has not developed or implemented a written hazard communication program.   ABATEMENT NOTE:  A written program shall include a description of how the criteria for the following will be met:  1.        Labeling and other forms of warnings;  2.        Safety Data Sheets;  3.        Employee information and training.      Additionally, a list of hazardous chemicals known to be present in the workplace must be compiled.  Methods used to inform employees of the hazards associated with non-routine tasks and the informing of contractors of workplace hazards, including a description of the labeling system used in the facility and the availability of safety data sheets must also be addressed.  The written program must be made available upon request.
