Bessemer Concrete, LLC: On or about September 15, 2020 at the address of 205 West Washington Street, New Castle, PA: Employees had not received ladder safety training which covered the proper use, care, maintenance, and inspection of ladders. Employees utilized ladders that included but were not limited to a 6-foot Werner step ladder to access the loads being strapped on the flat bed trailer of the Semi.
