29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. On or about October 08, 2020, and times prior thereto: a. In the machine shop, employees were exposed to struck-by and caught-by hazards when operating a Bullard brand 64" VTL lathe without a guard to protect the operator from the rotating chuck. b. In the machine shop, employees were exposed to struck-by and caught-by hazards when operating a King brand 84" VTL lathe without a guard to protect the operator from the rotating chuck. c. In the machine shop, employees were exposed to struck-by and caught-by hazards when operating a Lagun brand AT 2180G lathe without a guard to protect the operator from the rotating chuck. d. In the machine shop, employees were exposed to struck-by and caught-by hazards when operating a Lagun brand LS 28100 lathe without a guard to protect the operator from the rotating chuck. e. In the machine shop, employees were exposed to struck-by and caught-by hazards when operating a Lagun brand LS 36120 lathe without a guard to protect the operator from the rotating chuck.

On or about September 24, 2020, in the machine shop, the employer did not report a work related incident that resulted in a patient hospitalization, after an employee's right hand was injured after it got caught in a vertical lathe.
