29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to the environment to control or eliminate any hazards or other exposure to illness or injury: North side of the building: On or about October 7, 2020 an employee was exposed to a fall hazard, while attaching metal strips to the building from the basket of a telescopic aerial lift, without using fall protection. Geovanny Lopez was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1400759, citation number 1, item number 1, and was affirmed as a final order on August 8, 2019, with respect to a workplace located at 23755 U.S. Highway 281, San Antonio, TX 78258.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.
