29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Yarn Prep Department (IBCT Twister Operations): On or about October 03, 2020, and at times prior thereto; the employer exposed employees to caught-in, struck-by and amputation hazards in that the employer failed to ensure employees utilized the hazardous energy control procedure when employees were engaged in servicing and maintenance activities, including but not limited to clearing yarn jams with their hands in close proximity to the bottom moving spindle.
