29 CFR  1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    (a) On or about 10/08/2020 - Chemical storage, machining, and finishing areas, employees were exposed to chemical hazards when the employer did not provide and require employees to use protective equipment such as, but not limited to, safety glasses, goggles, or face shield when dispensing or working with corrosive chemicals such as, but not limited to, EPS 4000 Electropolish, Total Cell Cleaner, Sulfuric acid 66 BE NSF, and Raytech Compound M.  (b) On or about 10/08/2020 - Swiss machining area, employees were exposed to struck-by hazards when the employer did not provide and require employees to use protective equipment such as, but not limited to, safety glasses or face shield when operating the two Delta variable speed grinders with a Standard Abrasives surface conditioning or polishing and buffing wheel to remove burrs from spinal implant parts.
