29 CFR  1926.100(a):  Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.  On or about October 7, 2020, at an exterior side of building A, an employee in an extensible boom lift basket did not wear a hard hat  when there was possible danger of impacting building surfaces.  The employee was exposed to head injuries when working in an aerial lift basket under building surfaces.

29 CFR  1926.501(c)(1):  When employees were exposed to falling objects, the employer did not have each employee wear a hard hat and did not erect toeboards, screens, canopy structures or barricades to prevent objects from falling from higher levels.   On or about October 7, 2020, at building A, the employer did not erect toeboards, screens, canopy structures or barricades to prevent objects from falling from higher levels at areas including but not limited to the following:   a. 4th floor overhead of stair case for 3rd floor b. Building A, 4th floor, wall openings at the south side c. Building A, southside stair landings  Employees were exposed to struck-by hazards from objects falling overhead.

On or about October 7, 2020, at the 4th Floor Building A, an unmarked electrical extension cord powered other electrical extension cords to power a circular saw.  Employees were exposed to electrical hazards when using unmarked electrical extension cords.

29 CFR  1926.405(g)(2)(iv):  Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.  a.  On or about October 7, 2020, at the 4th floor Building A, an electrical extension cord that lacked strain relief, powered an electrical extension cord that powered a circular saw and exposed employees to electrical hazards.  b.  On or about October 7, 2020, at the 1st floor Building A, temporary power receptacle, an electrical extension cord lacked strain relief and powered other electrical extension cords and exposed employees to electrical hazards.

29 CFR  1926.502(b)(2):  Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high.  a.  On or about October 7, 2020, building A, southside of the 4th floor, a wall opening lacked a midrail.  An employee who worked near the wall opening was exposed to falls greater than 6 feet to a lower level.  b.  On or about October 7, 2020, building A southside stairwell, stair landing between 2nd and 3rd floor, a midrail was not installed between the top edge of the wall and the floor, at a height of at least 21 inches along the length of the wall opening.  Employees were exposed to falls greater than 6 feet to a lower level at the stair landing.
