29 CFR  1926.451(g)(4)(i):  Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews.   On or about October 7, 2020, at the north end of building B, the employer did not ensure that guardrail systems were installed along all open sides and ends of a 6 tier supported scaffold platform where employees worked.  This exposed employees to falls greater than 10 feet to a lower level.

29 CFR  1926.451(g)(4)(xv):  Crossbracing used in place of a midrail was unacceptable because the crossing point of two braces was not between 20 inches (0.5 m) and 30 inches ((0.8 M) above the work platform or as a toprail crossing point of two braces were not between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform.  The endpoints at each upright were more than 48 inches (1.3 m) apart.   On or about October 7, 2020, at the north end of building B, a 6th tier supported scaffold platform lacked a toprail at a height between 38 inches and 48 inches above the work platform and exposed employees to falls greater than 10 feet to a lower level.

29 CFR  1926.451(h)(1):  Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.    On or about October 7, 2020, at the north end of building B, at the 6 tier supported scaffold platform where employees worked there were no toeboards, screens, guardrail systems, or debris nets, catch platforms, or canopy structures to contain or deflect falling objects.  Employees working at lower levels were exposed to falling objects.

29 CFR  1926.501(c)(1):  When employees were exposed to falling objects, the employer did not have each employee wear a hard hat and did not erect toeboards, screens, canopy structures or barricades to prevent objects from falling from higher levels.    On or about October 7, 2020, at building B, the employer did not erect toeboards, screens, canopy structures or barricades to prevent objects from falling from higher levels including, but not limited to the following:   a.  Wall opening  at the stair landing between the 4th and 3rd floors. b.  Wall opening at the stair landing between the 3rd and 2nd floors.  Employees were exposed to struck-by hazards from objects falling overhead.

29 CFR  1926.1052(c)(12):  Unprotected sides and edges of stairway landings were not provided with guardrail systems that meet the criteria contained in Subpart M of 29 CFR 1926.    On or about October 7, 2020, at a stairwell of building B, the employer did not ensure unprotected sides of a stairway landing were provided with a guardrail system.  A stair landing between the 2nd floor and the 1st floor did not have a guardrail at two unprotected sides and exposed employees to falls greater than 6 feet to a lower level.
