29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  a) On or about October 6, 2020, and at times prior, the employer did not ensure that work rests on grinding machinery were adjusted closely to the wheel with a maximum opening of one eighth inch. Employees were exposed to struck-by hazards, in that an 8" bench grinder, model 33-BG8, was used while the work rest was not adjusted properly. The distance between the abrasive wheel and the work rest was three eighths of an inch.       Abatement Note: Abatement certification is required for this item. 29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    a) On or about October 6, 2020, and at times prior, the employer did not ensure that the adjustable tongue on grinding machinery were adjusted closely to the wheel with a maximum opening of one fourth inch. Employees were exposed to struck-by hazards, in that an 8" bench grinder, model 33-BG8, was used while the tongue guard was not adjusted properly. The distance between the abrasive wheel and the tongue guard was 1.25".       Abatement Note: Abatement certification is required for this item. 29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed:  a) On or about October 6, 2020, and at times prior, Aspen Wall Wood, Inc. did not ensure that sprocket wheels and chains, 7 feet or less above floors, were enclosed. Employees in the area of the knife grinder in the sharpening room were exposed to amputation and/or caught between hazards where chains and sprockets were not guarded.  Aspen Wall Wood, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1309508, citation number 1, item number 1c, and was affirmed as a final order on June 20, 2018, with respect to a workplace located at 31405 County Road S, Dolores, CO 81323.
