29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Exterior Entrance, On the ground:  An employee was exposed to head injuries while walking below a steel worker in gage in edge work activities. The employee was not protected by a hard hat or protective helmet, on or about 10/6/20.  b) Exterior Front: An employees was exposed to head injuries while smoking in front of the building without the use of head protection, and employees were working above, on or about 10/6/20.   c) Building Interior-4th floor: employees were expose to head injuries while a plumber was working on a 8 foot step ladder, and co-workers were standing underneath, on or about 10/6/20.  d) Building Interior-4th floor: An employee was exposed to head injuries while working/walking without head protection and other employees were working above, on or about 10/6/20.
