On or about October 12, 2020, and at times prior thereto, employees were working in a sewer line manhole, a permit required confined space, without testing/monitoring the internal atmosphere of the space, exposing the employees to hazards such as, but not limited to, flammable gases, oxygen deficient atmosphere, or toxic air contaminants.
