29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:  At the Northside of building:  On or about October 13, 2020, an employee was exposed to a fall hazard from a pump jack scaffold that did not have a ladder or other means to access the work platform, while installing siding board to the exterior of the building.

29 CFR  1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary:  At  the Northside of building:  On or about October 13, 2020,  employees was exposed to a fall hazard, from a pump jack scaffold that was secured to the roof with wooden boards, while installing siding to the exterior of the building.
