29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.    a) Building Exterior Back: Employees were using pipe scaffold frame to access the scaffold, exposing employees to a fall of approximately 6 feet to the ground surface, on or about 10/6/20

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) Building Interior 4th floor Back side:  An employee was exposed to a fall hazard of approximately 60 feet while working at the edge without the use of personal fall arrest system, guardrail system, safety net system, on or about 10/6/20  c) Building Interior 4th Floor front and left side:  Employees were exposed to fall hazard of approximately 60 feet while working/walking on the 4th floor where section on the floor were without a guardrail system, safety net system or other means of fall protection, on or about 10/6/20.  b) Building Interior 5th Floor:  Employees were exposed to a fall hazard of approximately  10-12 feet while the mezzanine was not protected by guardrail system, safety net systems, or any other means of fall protection, on or about 10/6/20.
