29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a) Building Interior-4th floor: Employees were expose to head injuries while a plumber was working on a 8 foot step ladder, and co-workers were standing underneath, on or about 10/6/20.  b) Building Exterior ground level: An employee was exposed to head injuries while smoking at the ground level without head protection, on or about 10/6/20.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) Building Interior 4th Floor front and left side:  Employees were exposed to fall hazard of approximately 40-60 feet while working/walking on the 4th floor where section on the floor were without a guardrail system, safety net system or other means of fall protection, on or about 10/6/20.
