29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about August 28, 2020, the employer did not report within 24 hours the amputation injury of an employee who received a fingertip amputation injury to her right index and middle fingers while operating a food processor machine at her workplace. OSHA was not notified of the amputation injury until on or about October 5, 2020, via the OSHA online injury reporting webpage.
