29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Exterior of Building-ground level: Employee was exposed to head injury while working on the ground level without the use of protective helmets, on or about  10/6/20.  b) Exterior of Building-Pump Jack Scaffold: Employee was exposed to head injury while working on a Pump Jack Scaffold without the sue of  protective helmets, on or about 10/6/20.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end.  a) Building Exterior: An employee was exposed to a 12 foot fall hazard while engaged in activities from a pump jack scaffold that was 24 feet long. The end of the platform was not designed and installed so that the cantilevered portion of the platform was able to support employees without tipping, the pump jack scaffold had an 4  foot overhang over the support, on or about 10/6/20.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  a) Building Exterior-left side: An employee was exposed to a fall hazard while accessing a pump jack scaffold that was not provided with proper scaffold access. Employee was using the pump jack scaffold post to climb down from the scaffold, on or about 10/6/20.
