In the facility at 90 Avocado St Springfield, MA: On or about October 8, 2020, an employee working in the warehouse operated a Crown RR 3000 electric stand up forklift and Crown MP 3000 electric pallet jacks on a daily basis prior to being trained.

29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: In the warehouse at 90 Avocado Street Springfield, MA: On and before October 7, 2020, an employee working in the warehouse used a Crown 3300 electrical powered industrial truck daily and the truck had mechanical deficiencies including but not limited to worn tires, hydraulic system leaks and frayed wires covered with electrical tape.

29 CFR 1910.178(q)(7): Industrial trucks were not examined, at least daily or before each shift if used on a round-the clock basis, before being placed into service. In the facility at 90 Avocado St. Springfield, MA: On and before October 7, 2020, an employee operated five electrical industrial trucks daily including a Crown 3300 stand up forklift, a Crown rider/sit down forklift and several Crown MB3000 and Yale pallet jacks without an examination before each shift. The industrial trucks had worn tires, hydraulic system leaks and frayed wires covered with electrical tape.
