Worksite: The employer did not ensure that the All-Terrain forklift powered industrial truck (PIT) used to lift plywood to the roof was operated by a person competent to operate such equipment safely.

29 CFR  1910.178(m)(5)(iii): Powered industrial truck(s), with the operator dismounted and within 25 feet of the truck still in his view, did not have the load engaging means fully lowered, controls neutralized, and the brakes set to prevent movement.  Worksite: The employer did not ensure that the All-Terrain forklift powered industrial truck (PIT) used to lift plywood to the roof was attended or still in his view while the load was raised to the roof.

Worksite: The employer did not ensure that the employees who were working on the residential roof were protected from fall hazards above 6 feet by the use of guardrails, safety nets or personal fall arrest systems.  DGO Construction, LLC was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection Number 1461003, Citation Number 1, Item Number 2, and was affirmed as a final order on 03/27/2020 with respect to a workplace located at 43 George Street, Seymour, CT.  DGO Construction, LLC was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection Number 1329457, Citation Number 1, Item Number 2a, and was affirmed as a final order on 09/25/2018 with respect to a workplace located at Lot 50 White Tail Lane, Shelton, CT.
