29 CFR 1926.451(b)(8): At all points of a scaffold where the platform changes direction, such as turning a corner, the platform that rests on a bearer at an angle other than a right angle was not laid first, and platforms which rest at right angles over the same bearer shall be laid second, on top of the first platform. On or about October 7, 2020 employees performing stucco work from scaffolding were exposed to fall hazards because at all points of the scaffold where the platform changes direction the platform that rests on a bearer at an angle other than a right angle was not laid first.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. On or about October 7, 2020 employees performing stucco work from scaffolding were exposed to fall hazards in excess of 10 feet with no ladder attached to the scaffold.
